Merge worked/confirmed in DXCC/WAS/WAC/ITUZ/CQZ

The existing display of 3 columns of "worked, confirmed, needed" makes
it very hard to visualize worked but not confirmed items. Clean this up
by harmonizing it with the call roster visual language of using shadow
boxes, solid, and unhighlighted entries. Use the same colors as used
in CR to keep the UX experience consisten across new (CR) vs old
(reports) code.

Paul Traina 8fa9c14c7c Change LOTW filtering behavior in CR 
Roster was treating LOTW filtering as "advisory" if someone had
previously set LoTW filtering to "show me nothing older than 3 months"
they would still get older stations but with "?" -- this is inconsistent
with every other filter, and is annoying--a station that once uploaded
to LoTW 8 years ago shouldn't be considered a LoTW station. Fixes this.
